By booking this appointment, I acknowledge and confirm the following:

I confirm, Makeup Frenzi Co, will not be responsible or liable if the result of the service is not as expected as it should be. I confirm that I will follow the regimen and the suggested follow-ups of the artist in maintaining and treating the service provided.

I am allowing Makeup Frenzi Co to apply necessary products as part of the service. I understand that the result of this chemical may vary from one person to another.

I consent Makeup Frenzi Co to take photographs of the provided service.

I consent Makeup Frenzi Co to share the photograph to social media for marketing campaigns or testimonials. I have read this whole document and I accept the terms indicated above.

We want to give you the time and attention you deserve. If you must cancel or reschedule your appointment, please give at least 24 hours notice. Appointments not cancelled within the 24 hours forfeits total deposit. A new appointment and deposit fee must be made to secure a new booking.

At the time of booking, a 50% deposit fee is required. Deposits can be made via direct bank deposit or web payment. This deposit will be deducted from your final bill on the day of your service. Deposit fees are non-refundable.
